A bit over 3 months ago, someone posted on /r/ledgerwallet that he sent 0.5 BTC from a wallet containing 27.5 BTC using his Ledger, but the UTXO with the remaining 27 BTC ended up on a wrong address that he cannot access. That's a bit over 1.5M USD.
It seemed unlikely to be a simple mistake as the guy appeared to be very knowledgeable about the inner workings of Bitcoin. He used custom scripts to check a lot of possible addresses with different derivation paths.
The last post on his account was 3 months ago, saying that he is trying to figure out with Ledger support what happened and that he will post an update, which never happened.
